Rectangular double-sided form: one side shows a female figure in a long patterned dress with a neckline, wearing a helmet with a plume, holding a sword in one hand and a roll of paper in the other, with shoes and a plant ornament at the bottom. The other side shows a male figure in armor resembling Roman legionary or Greek hoplite armor, wearing a similar plumed helmet, holding a shield bearing two crossed swords and a crown and a javelin in the other hand; a sword is girded at his side, sandals on his feet, and a plant ornament at the bottom. 18th–19th century, Silesia Province 1742–1918.
